In accordance with the following terms, conditions and regulations governing exhibits of HFSA at the Phoenix Convention Center (PCC), October 9-12, 2026, the undersigned hereby makes application for exhibit space, which application, when accepted by HFSA, becomes a contract.
Terms and conditions listed in the Exhibitor Prospectus, as well as those conditions under which exhibit space at the Phoenix Convention Center (PCC) is leased to HFSA, are a part of this contract.

Booth Proximity Limitations: Must be island booth not adjacent to major competitors Bridge Bio, Astra Zeneca, Pfizer and others if possible

Payment Type: Invoice – Wire transfer information will be included in the invoice.
Payment:
A 3% fee will be added to credit card charges $5,000 and over. Credit card fees cannot be refunded in the event of a cancellation. Invoice payment must be received no later than 30 days after invoiced and no later than June 30, 2026.
Cancellation Policy:
If cancellation of space is made on or before July 31, 2026, 60% of the booth fee will be refunded. Credit card fees cannot be refunded in the event of a cancellation. No refunds whatsoever will be made on cancellations or requests to reduce booth size after July 31, 2026.
